About This Position:
As a pharmacist in a decentralized role, you are responsible and accountable for the provision of safe, effective, and timely medication therapy with a goal of optimizing patient outcomes. The pharmacist will provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states.
This position will provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting with physicians, nurses, and other health care personnel. The decentralized pharmacist serves as the primary pharmacy resource for designated units, providing direct patient care while supporting medication distribution, regulatory compliance, and interdisciplinary collaboration.
The pharmacist may be asked to participate in clinical research and practice advancement projects, precept pharmacy learners, as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes pharmacy practice and patient care outcomes.

The Pharmacy Services Department at Lee Health is dedicated to optimizing patient outcomes through interdisciplinary medication management, supported by a team of over 400 staff members. Pharmacists provide care across four adult acute care campuses and one pediatric campus, with a Lee Community Pharmacy embedded in each hospital to help streamline transitions of care after discharge.

What This Position Requires:
Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates.
Certification: BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.
License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure Other: Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
