About This Position:

As an LPG Clinical Pharmacy Specialist, you will be integrated into primary care with a focus on improving patient outcomes through assuring optimal prescribing practices consistent with evidence-based literature and treatment guidelines. You will:

  • Conduct medication use evaluations, and support patient access and adherence to therapy.
  • Utilize and leverage analytic data to determine which patients will benefit from interventions, medication education and collaborate with primary care providers and specialists in the care of these patients.
  • Perform detailed medication reconciliation.
  • Conduct virtual or face to face visits to provide comprehensive medication management.
  • Provide recommendations to providers regarding utilization of high-cost medications.
  • Participate in the development of quality measures and quality improvement initiatives.
  • Provide educational presentations to providers.
  • Provide technician supervision to ensure accurate and timely submissions of prior authorizations

About our Pharmacy Team:

The Pharmacy Services Department at Lee Health is dedicated to optimizing patient outcomes through interdisciplinary medication management, supported by a team of over 400 staff members. Pharmacists provide care across four adult acute care campuses and one pediatric campus, with a Lee Community Pharmacy embedded in each hospital to help streamline transitions of care after discharge.

Lee Health is nationally recognized for excellence in pharmacy practice, becoming the sixth recipient of the American Society of Health-System Pharmacists Center of Excellence (COE) in Medication-Use Safety and Pharmacy Practice Certification and the first multi-site health system in the United States to earn this distinction. This elite certification reflects the highest levels of pharmacy practice, patient care, and medication-use safety, reinforcing the quality and impact of services provided to the community.

Pharmacy services also extend across a variety of settings, including the Lee Health Cancer Institute, Primary care clinics, a hybrid emergency department, skilled nursing facilities, home infusion services, and our Employee Health Plan Pharmacy.

What This Position Requires:

Education:Pharm.D. from College of Pharmacy Accredited by American Association of Colleges of Pharmacy or equivalent clinical experience (to be evaluated on individual basis).

Experience:Two years of Clinical Pharmacy experience required, Three years preferred. Completion of pharmacy residency program preferred.

Certification:Board certification in area of specialty practice or obtained within 18 months of hire or start date in the position.Ambulatory Care Pharmacy Specialty Certification (BCACP) Preferred.

Board certification is not required for the Skilled Nursing Facility.

BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.

License:State of Florida Pharmacist License required. Consultant Pharmacist license required for specialists working in the Skilled Nursing Facility.

Other:Strong communication and interpersonal skills, data entry skills, analytical and advanced math skills required. Ability to cope with confrontation. Ability to assist in the supervision and education of staff pharmacists and technicians. Specialty expertise in the Clinical Pharmacy Specialist area of practice (currently Critical Care, Internal Medicine, Infectious Diseases, Pediatrics, Ambulatory Specialty Clinics or Skilled Nursing Facility Care).
