Gulf Coast Medical Center is seeking an experienced Medical Social Worker with supervisory experience, or current Medical Social Worker with a Master's degree in social work and five years of work experience in acute care.
Responsible for the scheduling, supervision, training and development of the Discharge Planning Case Managers and Medical Social Workers, controlling workflow and facilitating smooth operational functioning. Determines patient's discharge need, appropriate level of care, and plan of care. Assures coordination of resources to meet patient needs, and facilitate movement across the continuum in an appropriate and cost-effective manner. Demonstrates effective judgment and problem resolution by recognizing problems, objectively evaluating facts, and reaching sound judgment.
