The Clinical Coordination Supervisor provides leadership and oversight for Case Managers while maintaining active involvement in clinical coordination. This role is responsible for scheduling, supervising, training, and mentoring Case Managers, ensuring smooth workflow, and promoting consistent, high-quality patient care. The Supervisor applies expertise in assessment, planning, implementation, coordination, monitoring, and evaluation of services to meet individual health needs across the care continuum.
By fostering effective communication, optimizing resources, and promoting quality, cost-effective outcomes, the Clinical Coordination Supervisor ensures excellence in patient care delivery. The role requires sound clinical judgment, problem-solving, and decision-making skills, along with the ability to mentor staff and support continuous performance improvement.
Supervise, schedule, and mentor Case Managers to ensure effective workflow and professional development.
Assess, plan, implement, and evaluate patient care coordination across diverse care settings.
Communicate with patients, families, and healthcare providers to connect individuals with appropriate services and resources.
Promote quality outcomes through effective utilization management and resource optimization.
Collaborate with interdisciplinary teams to facilitate safe and timely transitions of care.
Monitor and analyze workflow to identify opportunities for improvement and implement solutions.
Demonstrate effective problem-solving by recognizing issues, evaluating facts, and applying sound judgment.
Ensure compliance with organizational policies, regulatory requirements, and accreditation standards.
Education
Associates Degree in Nursing required; BSN strongly preferred (or actively in progress).
Experience
Minimum of five (5) years of clinical nursing experience required.
Previous case management or leadership experience preferred.
Licensure
Current, active RN license in the State of Florida required.
Certifications
Certification must be obtained within three (3) years of employment(CCM, ACM, or NACCM).
