Minimum to Midpoint Pay Rate:$18.64 - $23.30 / hour

Summary

Do you have a passion for helping individuals overcome barriers and access life-changing care? Join our team as a Substance Use Disorder (SUD) Navigator, where youll guide patients through the path to recovery with compassion, respect, and evidence-based support.

What Youll Do:
As a Navigator, you will connect patients with medication-assisted treatment (MAT), harm reduction services, and other vital resources. Youll serve as a trusted advocateoffering warm handoffs to providers, arranging appointments, and ensuring seamless access to outpatient clinics, mental health services, shelters, primary care, and residential treatment programs.

Using motivational interviewing and a non-judgmental approach, you will:

  • Conduct brief assessments and identify patients with SUD or co-occurring mental health disorders.

  • Provide follow-up care, appointment reminders, and help patients overcome obstacles such as transportation, medication access, and insurance coverage.

  • Promote harm reduction strategies based on each patients goals, preferences, and life circumstances.

  • Build relationships with community service providers to expand available resources.

  • Advocate for a culture of low-threshold, stigma-free care in hospital settings.

Requirements

Education Requirements:High School or equivalent

Experience Requirements: 1-2 years' experience in Community Outreach or associated role. Experience connecting individuals and families with community resources/needs.

State of Florida Licensure Requirements: Valid Florida drivers license

Certifications Requirements: BLS and First Aid Certification within 90 days of hire, must pass background and fingerprinting checks.

Other Requirements: Demonstrated competency through training and experience in the performance domains of: Advocacy, Mentoring, Recovery Support, Professional Responsibilities, Networking, and Navigating.
