The Pharmacy Clinical Specialist for Cardiology, with a focus on Heart Failure, is responsible and accountable for the provision of safe, effective, and timely medication therapy with a goal of optimizing patient outcomes. This role leads the heart failure program and collaborates with multidisciplinary teams to improve the care of both inpatients and outpatients suffering from heart failure and related cardiovascular conditions. Through various assignments within the pharmacy department, specialist provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states.

Pharmacy Clinical Specialists proficiently provide direct patient-centered care and integrated pharmacy operational services in a decentralized and ambulatory practice setting with physicians, nurses, and other health care personnel. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management within specific patient care services and to ensure medication management is integrated throughout the institution. In these clinical roles, Pharmacy Clinical Specialists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized care to the patients in their assigned areas.

Pharmacy Clinical Specialists serve as pharmacy department resources and liaisons to other departments, hospital personnel, or external groups. The specialist also conducts clinical research and practice advancement projects, precept pharmacy learners, as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes pharmacy practice and patient care outcomes. The role is critical to advancing the heart failure program and optimizing the pharmacological treatment of heart failure, in line with current evidence-based practices and institutional goals.

Pharm.D from College of Pharmacy Accredited by American Association of Colleges of Pharmacy or equivalent clinical experience (to be evaluated on individual basis).

Additional Requirements

A Bachelor or Doctor of Pharmacy degree plus 2 years of post-graduate training in an accredited pharmacy residency program and specialized pharmacy residency program in cardiology/heart failure care preferred

OR

A Bachelor or Doctor of Pharmacy degree plus 1 year of post-graduate training in an accredited pharmacy residency program plus 3 years of clinical practice with focus in cardiology/heart failure carepreferred.


State of Florida Licensure Requirements

Consultant Pharmacist license required for specialists working in the Skilled Nursing Facility.

Board certification in area of specialty or obtained within 18 months of employment required. Cardiology Pharmacy Specialist Certification (BCCP) preferred.

Other Requirements

Strong communication and interpersonal skills, data entry skills, analytical and advanced math skills required. Ability to cope with confrontation. Ability to assist in the supervision and education of staff pharmacists and technicians. Specialty expertise in the Clinical Pharmacy Specialist area of practice (currently Critical Care, Internal Medicine, Infectious Diseases, Pediatrics, Ambulatory Specialty Clinics or Skilled Nursing Facility Care).
