The Transitions of Care Pharmacist plays a vital role in supporting safe and effective medication management as patients transition between hospital, home, and outpatient settings.
Key Responsibilities:
Identify and resolve medication reconciliation discrepancies at patient discharge.
Collaborate with physicians, nurses, case managers, and community providers to optimize medication therapy.
Provide patient and caregiver education to promote medication understanding and adherence.
Facilitate discharge planning by:
Preparing accurate and complete medication lists.
Coordinating access to prescriptions.
Addressing barriers such as cost, access, or side effects.
Support quality and safety initiatives aimed at reducing readmissions and improving patient outcomes.
Serve as a resource for staff, students, and residents on best practices related to transitions of care.
Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates.
Certification:BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.
License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
Other: Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
