Minimum to Midpoint Pay Rate:$51.54 - $67.01 / hour

**Eligible candidates can receive up to $7,500 in relocation!**

The Transitions of Care Pharmacist plays a vital role in supporting safe and effective medication management as patients transition between hospital, home, and outpatient settings.

Key Responsibilities:

  • Identify and resolve medication reconciliation discrepancies at patient discharge.

  • Collaborate with physicians, nurses, case managers, and community providers to optimize medication therapy.

  • Provide patient and caregiver education to promote medication understanding and adherence.

  • Facilitate discharge planning by:

    • Preparing accurate and complete medication lists.

    • Coordinating access to prescriptions.

    • Addressing barriers such as cost, access, or side effects.

  • Support quality and safety initiatives aimed at reducing readmissions and improving patient outcomes.

  • Serve as a resource for staff, students, and residents on best practices related to transitions of care.

Requirements

Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).

Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates.

Certification:BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.

License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.

Other: Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
