Facilitates a timely, integrated and coordinated approach to the care of patients. Uses a multidisciplinary approach and facilitates communication and collaboration between the patient, staff and referring physician offices as well as ancillary areas of the hospital system. Serve as a liaison for Orthopedic patients and their family throughout the entire care continuum: pre-surgery, pre-admission, In-patient, Post-op and Post Discharge with specific responsibilities at each of these points of care. Shares responsibility of auditing orthopedic related quality data, coordinating various Ortho Service meetings and progression of care rounds and leads or participates in various projects, clinical presentations and other activities as assigned. Develops, presents, coordinates and evaluates educational activities. Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.
