At Lee Health, were looking for a dedicated and empathetic professional to join our team in a vital role that blends clinical assessment, discharge planning, and compassionate care coordination.
What You Will Do:
In this meaningful role, you'll be the bridge between hospital and post-acute careassessing patients transferring from Lee Health facilities or other care settings and ensuring they meet Medicare or other payor source requirements. But this job is about more than paperworkits about people.
Youll partner closely with patients and their families to guide them through the discharge process, provide emotional support, and connect them with the resources they need to thrive beyond our walls. From needs assessments and crisis intervention to counseling and community referrals, you'll help patients maintain their highest level of independence and quality of life.
