JOB SUMMARY:

  • This position is responsible for providing leadership for the nurse navigation team, which includes Registered Nurses, Social Workers and Community Health Coordinators. The manager, Nurse Navigation oversees the day-to-day operations of care management provided to value based care patients. This oversight includes front line staffs assessment and evaluation of patients' needs development of care plans, interdisciplinary collaboration, productivity and outcomes. Additionally, this position will work closely with the System Director of the Center for Care Transformation to redesign the commercial payor focused work of the department. Examples include addressing health equity and gaps in care, ensuring compliance with the provider plan of care, and referring patients to resources such as behavioral health. The manager is responsible for ensuring that their staff is providing the right care at the right time and place at the right cost. The manager will assist in development of and implement enhanced patient centered care utilizing nationally recognized care standards and pathways. This position will support the coordination of community based patient care with multiple post-acute entities. The manager shall oversee the coordination of transitions for patients and care management interventions delivered in the home, hospital, office and SNF.

    ELEMENTS:

    Education Requirements: BSN required, MSN highly preferred

    Experience Requirements: 5 Years Care Management or Community Home Health Management

    required.

    State of Florida Licensure Requirements: RN license required

    Certifications/Registration Requirements: Care Management Certification (CCM, ACM or similar). If not certified, must obtain certification within 12 months.

    Other: Experience in coordinating care with multiple teams in the post-acute space. Leads and ensures the safety and expertise of their team. Coordinates the in home and remote care for value based and LPG patients.
