As a Case Manager, you will play a vital role in guiding patients through their healthcare journey. You will use your clinical knowledge, critical thinking, and understanding of community resources to ensure each patient receives safe, timely, and cost-effective care. Working as a key member of the multidisciplinary team, you will anticipate patient needs, coordinate services, and ensure smooth transitions across the care continuum.

Apply clinical expertise to determine medically appropriate levels of care using established criteria.
Partner with physicians, nurses, social workers, and other team members to develop safe and comprehensive discharge plans.
Coordinate services, resources, and referrals to support patients ongoing medical, functional, and social needs.
Evaluate patient progress and adjust plans to ensure quality outcomes and cost-effective care.
Facilitate communication across the care team to reduce barriers and ensure timely transitions.
Support utilization and quality management efforts to promote optimal patient care and organizational efficiency.
Education: Associates Degree in Nursing required; active pursuit of a BSN preferred.
Experience: Minimum of 3 years of clinical nursing experience. Prior experience in case management, discharge planning, utilization management, or working with community resources is highly valued.
License: Active RN license in the State of Florida.
Certifications: ACM, CCM, NACCM, or CMCN preferred.
Skills: Strong communication, collaboration, critical thinking, and the ability to multitask and prioritize in a fast-paced environment. Basic computer proficiency and sound documentation skills required.
