Summary
The Case Manager RN is a key clinical leader who combines expert nursing judgment with a deep understanding of utilization, quality management, and discharge planning. Serving as a vital link between patients, providers, and community resources, the Case Manager ensures that each patient receives safe, timely, and cost-effective care across the entire continuum. This role is both analytical and compassionatebalancing the science of care coordination with the art of patient advocacy to help individuals achieve their highest possible level of health and independence.
Key Responsibilities
Collaborate with the multidisciplinary team to develop and implement individualized care plans.
Apply evidence-based criteria to evaluate medical necessity, level of care, and appropriate resource utilization.
Facilitate smooth and effective patient transitions across all settings of care.
Identify barriers to care and connect patients with the right services and community resources.
Monitor outcomes, identify trends, and participate in quality and performance improvement initiatives.
Serve as a patient advocateempowering individuals and families to make informed decisions about their health.
Requirements
Education:
Associates Degree in Nursing required. Bachelors Degree in Nursing preferred or actively in progress.
Experience:
Minimum of 3 years of clinical nursing experience required.
Prior experience in case management, utilization review, discharge planning, or community resource coordination preferred.
Licensure:
Current and active RN license in the State of Florida.
Certifications:
Certification in Case Management (ACM, CCM, NACCM, or CMCN) preferred.
Skills and Competencies:
Exceptional communication and interpersonal skills.
Strong critical thinking, assessment, and analytical abilities.
Proficiency with electronic medical records and standard computer applications.
Ability to manage multiple priorities in a fast-paced, collaborative environment.
