The Case Manager RN is a vital member of the multidisciplinary care team, bringing strong clinical expertise and a deep understanding of evidence-based criteria to support safe, effective, and patient-centered care. This role applies the principles of utilization management, quality improvement, and discharge planning to ensure appropriate use of resources while promoting optimal health outcomes.
The Case Manager proactively anticipates patient needs, coordinates services across the continuum of care, and links patients and families with community resources to maximize independence and overall well-being. Serving as both a facilitator and patient advocate, the Case Manager helps create a seamless transition of care while balancing clinical excellence with financial stewardship.
Collaborate with providers, nursing staff, and the multidisciplinary team to develop individualized care plans.
Facilitate safe and efficient patient transitions across care settings, ensuring timely discharge planning and continuity of care.
Apply utilization and quality management principles to monitor appropriateness of care and optimize resource use.
Assess clinical, social, and financial factors that impact patient outcomes and provide interventions to address barriers.
Serve as a liaison between patients, families, providers, and community resources to promote patient-centered care.
Actively participate in quality initiatives and performance improvement projects.
Advocate for patients by ensuring access to necessary services and supporting long-term health maintenance.
Education
Associates Degree in Nursing required; BSN strongly preferred (or in active progress).
Experience
Minimum of three (3) years of clinical nursing experience required.
Experience in discharge planning, utilization management, or case management strongly preferred.
Knowledge of community resources and patient assessment skills desired.
Licensure
Current, active RN license in the State of Florida required.
Certifications
Case Management certification (ACM, CCM, NACCM, or CMCN) required within five (5) years of employment.
