The Post-Acute Care Manager collaborates with the community, as well as with acute and post-acute healthcare teams, to ensure timely triage and assessment of potential and current referrals for admission. This role involves conducting thorough clinical, social, functional, and financial assessments, using established criteria to evaluate the need for post-acute services. These services may include skilled nursing facilities, skilled nursing units, or home health care.
The Post-Acute Care Manager will engage with key stakeholders to provide consistent education on quality metrics, specialty programs, and the full range of post-acute services. Additionally, this role shares responsibility within the post-acute teams to support business growth and increase patient volumes. This includes developing and executing strategies to attract new patients, foster relationships with referral sources, and enhance the visibility of post-acute care services in both the community and the health system.
Furthermore, the Post-Acute Care Manager will work closely with the healthcare team to ensure smooth transitions for patients moving from community or healthcare settings to the appropriate level of care.
Education:Associates degree in nursing required. Graduation from a Diploma Nursing program.
Experience:3 Years Clinical Nursing Required. Previous experience in discharge planning, assessment skills and clinical social factors that affect patients functional status at discharge preferred. Previous Experience in Home Health or Skilled Nursing Facility/Unit preferred. Case Management or discharge planning required.
Certification:BLS (American Heart Association/Basic Life Support) required. PAC-RN certification preferred.
License:Florida Registered Nurse license required.
Other:Must be able to travel among campuses as needed. Must be able to work some weekends and holidays.
