This is a driving role visiting patients in the home.
Lee HealthsHospital at Home programservice brings hospital-level care to eligible patients at home, including daily visits, remote monitoring, and treatments. It helps patients recover comfortably while reducing hospital-related risks. The program reflects Lee Healths commitment to innovative, patient-centered care in Southwest Florida.
Responsible for the safe delivery and documentation of patient care provided through the nursing process of assessment, diagnosis, planning, implementation, and evaluation. Performs responsibilities in the five areas of nursing practice: clinician, teacher, researcher, leadership, and professional development utilizing critical thinking, management, problem solving, and differentiation of urgency. Responsible for nursing care of acute care patients in the home setting based on established clinical nursing practice standards subscribed to by the American Nursing Association and in accordance with the Florida Nurse Practice Act and other regulatory standards. The nurses practice is guided by the Code of Ethics for Nurses with Interpretative Statements, with the nurse integrating knowledge of patients age and cultural diversity into the provision of holistic patient care. The Hospital at Home Nurse contributes to the provision of quality nursing care through excellence, customer service, performance improvement, and mentoring to deliver positive patient outcomes and experiences. Collaborates within the interdisciplinary team to deliver effective and efficient patient care and supports the organizations mission, vision and values.
Education: Associate degree from an accredited school of nursing required; bachelors in nursing preferred.
Experience: Minimum of 2 years of nursing experience, progressive care. Emergency or intensive care preferred. Recent clinical experience within the last two years upon application preferred.
Certification: Proof of American Heart Association Provider Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification. ( current for a minimum of 90 days after hire date.)
License: Currently licensed RN Licensure in the State of Florida.
Other: Strong communication and interpersonal skills with the ability to flex and adapt to evolving environment and role.Computer, telephone, and customer service skills required.
