Work Type:Full Time, 80 hours bi-weekly (Exempt/Salaried Position)

Shift:Days, 8:00:00AM to 4:30:00PM (Flexible to meet patient and business needs)

This is a driving role, visiting patients in the home.

Minimum to Midpoint Pay Rate: $33.37 - $45.05 / hour

What You'll Do

As a valued member of Lee Health Home Health, you will:

  • Conduct comprehensive patient assessments, including OASIS documentation
  • Develop and manage individualized care plans based on clinical best practices
  • Deliver hands-on skilled nursing care in patients' homes
  • Educate patients, families, and caregivers to support recovery and independence
  • Coordinate care with providers and interdisciplinary teams to ensure seamless transitions
  • Supervise and collaborate with LPNs and Home Health Aides
  • Document care and patient progress to support quality outcomes

Available Coverage Areas

We are hiring for multiple Home Health territories across Southwest Florida:

  • Central Team: Fort Myers, Lehigh & Alva
  • North Team: North Fort Myers, Cape Coral & surrounding areas
  • South Team: South Fort Myers, Estero & Bonita Springs

Requirements

Education:Graduate of an accredited school of professional nursing.

Experience:Minimum one-year acute care experience required. Minimum two (2) years of cardiac experience preferred.

Certification:BLS (American Heart Association/Basic Life Support) Required.

License:Currently licensed RN in the State of Florida.

Other:Communication and interpersonal skills, basic computer skills, and customer service skills required.
