Work Type: Full Time, 80 hours bi-weekly (Exempt/Salaried Position)

Shift: Days, 8:00:00AM to 4:30:00PM (Flexible to meet patient and business needs)

This is a driving role, visiting patients in the home.

Minimum to Midpoint Pay Rate:$33.37 - $45.05 / hour

Hiring incentive up to $5,000 for qualified candidates

• Conduct comprehensive assessments, including OASIS, to establish tailored care plans.

Summary

Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Assists with directing and coordinating nursing care for patients based on established clinical nursing practice. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.

Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering weekends and team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization.

Requirements

Education: Graduate of an accredited school of professional nursing.

Experience: Minimum one-year acute care experience required. Minimum two(2) years of cardiac experience preferred.

Certification: BLS (American Heart Association/Basic Life Support) Required.

License: Currently licensed RN in the State of Florida.

Other: Communication and interpersonal skills, basic computer skills, and customer service skills required.
