Lee Healths Hospital at Home programbrings hospital-level care to eligible patients at home, including daily visits, remote monitoring, and treatments. This is a part-time position (36 hours bi-weekly)
Facilitates a timely, integrated and coordinated approach to the care of acute patients in the home setting (Hospital at Home).
Uses a multidisciplinary approach and acts as a liaison between the patient, staff and acute care hospitalist as well as ancillary areas of the hospital system. Identifies learning needs and content for educational programs.
Develops, presents, coordinates and evaluates educational activities.
Effectively performs in the role of clinician, facilitator, advocate, consultant and/or researcher.
Examples of specific functions performed in this role:
1) Performs both direct and indirect care for acute patients in the home setting with a high degree of skill and knowledge.
2) Coordinates care of the patient admitted to Hospital at Home with the collaboration of the admitting Hospitalist. Includes admission and care following discharge in the home.
3) Serves as liaison for Hospital at Home patients, with physicians and other departments, to help obtain/expedite recommendations from physicians and other health team members for timely and appropriate patient care.
4) Collaborates with medical, nursing and technical staff in data collection and statistical activities to facilitate problem identification and resolution to improve quality of care and to comply with identified standards.
5) Refers patients/family to appropriate resources/services, based on needs assessment.
6) Provides/oversees education, support and necessary resources for patients and their families, and coordinates appropriate referrals for those needed services.
7) Coordinates/oversees schedule of exams, procedures and appointments, etc., identifying potential gaps that are not being met. Maintains close contact with patient and family throughout the hospitalization and recovery phase.
8) Conducts educational classes/talks in collaboration with community outreach programs.
9) Coordinates programs for community education as well as hospital at home, hospital departments, and employees.
10) Reviews/audits clinical documentation to ensure accuracy and compliance. Collects and reports data to monitor and evaluate outcomes
Education: Bachelors degree in nursing required, Masters preferred.
Experience: Minimum of 3 years acute care clinical experience. (8 yrs acute experience in ED or more preferred )
Certification: American Heart Association Provider Basic Life Support (BLS) required.
License: Currently licensed or eligible for RN Licensure in the State of Florida.
Other: Strong communication and interpersonal skills with the ability to flex and adapt to evolving environment and role.
Other: Computer systems, telephone/video, and customer service skills required.
