About This Position:

At Lee Health, ourComplex Care Centersare dedicated to providing specialized, coordinated Transitional care for adults managing multiple or chronic medical conditions. Our Complex Care Centers at Cape Coral Hospital and at Lee Memorial Hospital brings together a multidisciplinary team to deliver advanced treatments, including infusion therapy, wound care, and provider-based services - all focused on improving outcomes and enhancing patient experience.

This unique care model helps reduce hospitalizations, streamline treatment plans, and ensure patients receive the right care at the right time.

Key Responsibilities:

  • Deliver safe, high-quality patient care using the nursing process (assessment, planning, implementation, evaluation)
  • Provide patient education and support throughout treatment
  • Collaborate with providers and interdisciplinary teams to coordinate care
  • Document care accurately in alignment with regulatory and organizational standards
  • Promote positive patient outcomes through evidence-based nursing practice

Performs and documents selected tasks such as the administration of treatments and medications in delivery of patient care under the guidance and discretion of the registered nurse. Assist the Register Nurse in collecting health data according to established policies. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.

Requirements

Education: Graduate of an accredited vocational school of nursing required.

Experience: None required. Qualified individuals will successfully complete the competency assessment.

Certification: Proof of American Heart Association Healthcare Provider Basic Life Support (BLS), current for a minimum of 90 days after hire date. 30-hour IV Course may be required based on specialty.

License: Currently licensed as an LPN in the State of Florida.

Other: Communication and interpersonal skills, basic computer skills, and customer service skills required.
