The Patient Outreach Medical Assistant performs concurrent and retrospective clinical chart reviews and abstracts data for the purpose of closing clinical quality measures. The information is submitted back to payors per the contracted methodology. These submissions support LPG quality initiatives (HEDIS, Medicare Star, etc.) and pay for performance or shared savings initiatives. This position provides indirect assistance to the physician, patient and care team by supporting patient adherence to the medical plan of care and/or evidenced based medicine guidelines to include all appropriate preventative and disease-specific screenings, lab/diagnostic testing and scheduled follow up appointments. The Patient Outreach Medical Assistant communicates with this population of patients via telephone, MyChart, or the preferred method of correspondence to support the patient adherence to the plan of care and/or evidenced based medicine guidelines and documents their intervention in the electronic health record.
Education:High School Diploma or GED Required. Completion of a medical assisting program or postsecondary medical assisting program accredited to prepare the medical assistant to take national CMA (American Association of Medical Assisting), CCMA (National Health Career Association), or RMA (American Medical Technologists) medical assistant certification exam preferred.
Experience:3 years (fulltime) recent clinical experience in Internal Medicine and/or Family Practice utilizing an electronic health record required.
Certification:AAMA (American Association of Medical Assisting), CCMA (National Health career Association), RMA (American Medical Technologists) preferred. BLS (American Heart Association / Basic Life Support) required (must be obtained within first 90 days of hire).

Other:Medical terminology, communication skills, and basic knowledge of computers required.
