Summary:
Facilitates a timely, integrated and coordinated
approach to the care of patients with surgical cardiac and thoracic disease. Coordinates
care for patients throughout the continuum of care from diagnosis to the
release from care. Uses a
multidisciplinary approach and acts as a liaison between the patient, staff and
referring physician offices as well as ancillary departments of the hospital
system. Identifies learning needs and content for educational programs.
Develops, presents, coordinates and evaluates educational activities.
Effectively performs in the role of clinician, facilitator, advocate,
consultant and/or researcher. The Nurse
Navigator will guide the patient/family through the complexities of the
healthcare process, working to eliminate barriers that might otherwise
adversely impact patient care/outcome and will help to coordinate the efforts
of the medical team to facilitate continuity of care and enhance patient
satisfaction.
