Come join our Population Health Team and help us build our new strength based social work model.
Assess, plan, implement, coordinate, counsel, monitor, and evaluate options and services to meet specific individual's health needs throughout the care continuum in various settings through use of available resources to promote quality cost-effective outcomes. The Longitudinal Care Advisor, along with other members of the Population Health team, will conduct comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identification of both gaps in care and barriers to attaining improved health.

3 Years clinical experience in medical or healthcare setting.
• 3 Years’ experience in acute-care care management.
Knowledge of clinical and social factors affecting patients’ functional and behavioral status and their ability to transition to or remain in an outpatient setting. Ability to conduction comprehensive assessments and utilize targeted solution-oriented treatments. Exceptional clinical, organizational, and critical thinking skills that lead to effective treatment planning and decision making. Excellent written, verbal and non-verbal communication skills with people of various ages, backgrounds, education levels and ethnicities. Proficient in basic computer operations, and Microsoft office applications. Ability to work collaboratively in a team environment managing multiple priorities using sound judgment. Uses effective coping mechanisms.
