This position is open to qualified RN's, LPN's, Social Workers and Therapy.

Responsible for gathering information related to patients being referred to Lee Health Home Health. The position is responsible to determine and document needs to assist field clinicians providing services to the patient. This position completes and facilitates the intake processes and provides support to the other team members. Teamwork, flexibility, and professionalism are central to the success of the team, and are expected in this role.


Education Requirements: Graduate of an accredited school of professional nursing

Experience Requirements: Minimum one year acute care experience, home health experience preferred, qualified individuals will successfully complete the competency assessment.

State of Florida Licensure Requirements: Currently licensed or eligible for RN licensure in the State of Florida;

Certifications/Registration Requirements: Active BLS certification

Other: Experience in Home Health or Rehabilitation Nursing. Case Management or Discharge Planning and Leadership experience preferred. Covers weekends, holidays and other team members as needed. Excellent customer services skills to meet the needs of external and internal referral sources.
