ED CASE MANAGER:

ESSENTIAL DUTIES and RESPONSIBILITIES:
As an ED Case Manager, you will:
Utilize critical thinking and problem solving skills to expedite discharge planning through assessment, referral and interdisciplinary planning for high risk populations.
Work collaboratively with the clinical team to provide emergent clinical interventions and crisis management along with discharge planning through assessment and counseling.
Link patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning.
Provide utilization management functions to ensure medical necessity and the appropriate setting for the patient.
JOB REQUIREMENTS:
Education:Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree within 5 years.
Experience:Minimum of 3 years clinical nursing experience. Previous experience in discharge planning, utilization management is a plus.
Training:On the Job
License:Currently licensed as an RN in the State of Florida.Require Case Management Certification, including CCM, ACM or NACCM after 5 yrs. employment (time exception provided for unsuccessful attempts).
Other:Strong communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task responsibilities.
Summary:
Emergency Department Utilization Care Managers perform utilization management functions to ensure medical necessity and the appropriate level of care. Assesses, plans, implements, coordinates, monitors, and regulates compliance options and services to meet an individual's health needs throughout the continuum. Collaborates through communication and available resources to promote quality cost-effective outcomes. Expedites linking patient with systems in the community in conjunction with ED MSW to provide them with resources, services and opportunities to achieve and maintain maximum functioning. Facilitates concurrent documentation in the electronic medical record to accurately reflect care, services and resources utilized in the delivery of patient care. Successfully meets requirements for productivity and accuracy related to all utilization management processes.
