Summary:

Assesses// plans// implements// coordinates// monitors// and evaluates options and services to meet an individual's health needs throughout the continuum in various settings through communication of available resources to promote quality cost-effective outcomes. The Clinical Case Manager provides discharge planning through assessment// referral and interdisciplinary planning for high risk population and also links people to systems that provide them with resources// services and opportunities to achieve and maintain maximum functioning.
