ESSENTIAL DUTIES and RESPONSIBILITIES:
As a Case Manager, you will work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with clients, their family, behavioral health, medical providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented. The Clinical Case Manager utilizes critical thinking and problem solving skills to provide discharge planning through assessment, referral and interdisciplinary planning for high risk populations.The Case Manager also links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning.
JOB REQUIREMENTS:
Education:Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree within 5 years.
Experience:Minimum of 3 years clinical nursing experience. Previous experience in discharge planning, utilization management a plus.
Training:On the Job
License:Currently licensed as an RN in the State of Florida.Require Case Management Certification, including CCM, ACM or NACCM after 5 yrs. employment (time exception provided for unsuccessful attempts).
Other:Strong communication and interpersonal skills, basic computer, analytical, grammar and spelling skills. Ability to effectively multi-task responsibilities.
